Error copying a voucher to create a reverse - wrong # columns

VO bank combo box not loaded on VO Batch form load
Symptoms:
When Codes VOBANK="Y" and you add a new batch, the bank combo box is blank, which gives you an error message saying 'invalid choice'. It should be loading the combo box.
Solution:
The combo box is loaded correctly but the edit routine fails when the voBank covalue is set to "Y". If voBank = "Y", added a blank bank string to the combo box to resolve the problem. In addition: . trying to copy a voucher and create a reversing voucher, a user might get the error "column name or number of columns does not match table definition". The insert has been fixed. See patch 50.019.001
